In order for teen to participate, a parent/legal guardian must sign a Release and Hold Harmless form and return form to the program location at least one week prior to the program start date.
Participants may be photographed/recorded and/or surveyed to capture the program's effectiveness. If you do not want your teen to be photographed/recorded and/or surveyed, please fill out the Audio/Visual Recordings Opt-Out form and/or the Surveys Opt-Out form.
